Job Description:
A company is looking for a Clinical Inbound Personal Care
Coordinator to assist patients in developing personalized care
plans and achieving health goals.
Key Responsibilities:
Engage with patients through various communication channels to
address health needs and schedule follow-ups
Facilitate care coordination by identifying resources and
developing goal-oriented interventions
Ensure accurate clinical documentation and compliance with
healthcare regulations and standards
Required Qualifications and Education:
Active COMPACT LPN license
Previous experience in care coordination, healthcare call centers,
or patient advocacy
Proficiency in electronic health records (EHR) and multiple
systems
Adherence to attendance policy and metric standards
